Can a Certificate of good standing be issued without a lodge name and number listed on the application?

I'm not aware of any jurisdiction that does that. Good standing means you are current on your dues - How to confirm that without saying where they are current ...

Why would a brother even want to do that? We carry dues cards or other documentation that tells what lodge we are a member of. That's to demonstrate that the lodge we are a member of isn't clandestine. Thus if you try this some will wonder if what you're trying to do is be healed. If you want to be healed go ahead and say that up front.

How can you be in good standing of a non-existent lodge?

When a lodge's charter is pulled its members become "members at large" who need to affiliate with some other lodge. Depending on the rules of a jurisdiction they might or might not be assigned a number. Temporary status.

If I understand the question correctly, the Brother wishes to either transfer his membership or affiliate as a plural member but has not yet decided with which Lodge to so do. If this is the case, the answer is yes. When he decides & is accepted, the Secretary of that Lodge will complete the return slip & send it to you.

If you do not know the name & number of the new Lodge just provide the CGS without it.
