In Pennsylvania you need to contact the Secretary of your former Lodge and begin the process with him. If you don't know whom that may be, go online and find the information on the Lodge website or contact your Grand Lodge Grand Secretary's office.
Here in Pennsylvania we have Liberty Lodge #505 in Port Allegany, PA. Lodges in PA are known as "F&AM", not "AF&AM", so it's possible someone from this Lodge put this up for whatever reason. Curious.....................
