This is what petitioning African Lodge No. 459 Boston would have looked like then.
The document is dated August 4, 1802, and addressed to the Worshipful Master and Wardens of African Lodge on behalf of John Lewey. The subscribers vouching for John Lewey are James Willis and Charles Mitchell. The document reads as such:

"To the Worshipful Master and Wardens of the African Lodge-----

John Lewey wishing to become a member of your honorable Society We the subscribers having known him for a length of time recommend him as a man of sober life and Conviction and a fit person to be admitted-------

    Signed{ James Willis
    Charles Mitchell

    Boston Aug. 4th 1802
