GL form MES-502

Im trying to find GL form MES-502 that is used to track EA/FC and where they are in their training/proficiency etc. It may do more than that. I cant find it and none of the secretaries Ive talked to have it. Its mentioned in the LIFE SW Book and Id like to use it or maybe make an electronic version of it to track our young brothers better. We checked on the GL Website so maybe the LIFE book is just outdated?

Brother, I believe this is an oudated part of the LIFE Program. I have ran into this many times when I am teaching it. There is a few other things that are outdated in there. I really wish our GL would take the time to update these books, along with the ALL program. Its just a shame that it is manditory that we use it yet it is so outdated.
