I'm not sure if you are looking for out-of-the-country projects too, but this one is the main project of my Grand Lodge. As we are only about 3,000 members in my GL, so all help is appreciated.

The Zinnendorfstiftung is a small nursing home for multiple or heavily crippled young people, run by a charitable trust. The patients don't need to have masonic connections.
They don't have a website in English, but maybe Altavista or Babelfish helps.
